Take Home Point #8: Avoid General Statements and Be Specific in Your Advice to the Patient

In the NAC-OSCE exam, it is important to provide the patient with the detail and quality of information that reflects your expertise as a physician.

For example, when you are counselling a patient with insomnia it is insufficient to just tell them that "drinking that much coffee is associated with many health risks, so please reduce your coffee intake. "What does this general statement mean?" Instead, be specific in your advice to the patient. Explain why so much coffee intake could be impacting their health and recommend how much coffee is safe for them to consume daily and when they should stop their caffeine consumption for the day: "I think the 5 to 6 cups of coffee a day are linked to the trouble you've been having falling asleep lately. Coffee contains a lot of caffeine which is a stimulant. It can over stimulate the brain. How would you feel about limiting your coffee intake to only 2 cups a day and enjoying them no later than 3pm. I think your ability to fall asleep will improve. Is this something you'd be open to trying?"

Another common example of vagueness often occurs in cases where the patient is asked to adopt a new diet or modify their current diet. Please note that it is not enough to just tell the patient that they "need to eat a healthier diet". What does this general statement mean? Again, be specific in your advice to the patient. Why should they eat a more balanced diet? What should they eat more of? What should they eat less of? Should they drink more water? Further, if you refer the patient to a nutritionist or dietician, explain the role that they will play in their care. Never assume that the patient knows what a nutritionist is.

So, avoid general statements and be specific in your advice to the patient and show the examiner that you have the ability and expertise as a physician to fully and accurately counsel your patient.
